This system operates through multiple levels of defense, including the pre-epithelial mucus-bicarbonate barrier, the epithelial cell layer capable of rapid restitution and proliferation, and a robust sub-epithelial microcirculation (Laine et al., 2008, Gastroenterology). Key molecular mediators in these pathways include prostaglandins (particularly PGE2 and PGI2), which stimulate mucus secretion and blood flow, and growth factors such as epidermal growth factor (EGF) and trefoil factor family (TFF) peptides that facilitate tissue repair (Tarnawski & Ahluwalia, 2012, Curr Med Chem). In disease states like peptic ulcer disease or NSAID-induced gastropathy, these protective mechanisms are overwhelmed or impaired (StatPearls, 2023). Therapeutic intervention often involves cytoprotective agents that mimic or stimulate these endogenous pathways to enhance healing and prevent mucosal injury (PubChem, 2024). These drugs, such as misoprostol or rebamipide, work by increasing mucus production, improving blood flow, and accelerating the repair of damaged tissue.
