Gastric mucus components synthesis and secretion refers to the multi-step physiological process by which the stomach produces and maintains its primary defense barrier against luminal insults. This barrier is a viscoelastic gel layer composed of water, electrolytes, and high-molecular-weight glycoproteins called mucins, predominantly MUC5AC (secreted by surface foveolar cells) and MUC6 (secreted by mucous neck cells) (PMID: 11709514). The synthesis of these components is heavily regulated by endogenous prostaglandins, specifically PGE2 and PGI2, which stimulate mucus and bicarbonate secretion while maintaining mucosal blood flow (StatPearls, 2023). In disease states such as peptic ulcer disease and NSAID-induced gastropathy, the synthesis of these protective components is compromised, leading to acid-mediated mucosal erosion (PMID: 15722384). Drugs such as Misoprostol, Rebamipide, and Teprenone are used to treat these conditions by enhancing the synthesis and secretion of gastric mucins and reinforcing the mucosal integrity (PMID: 11841214, PMID: 15155615).
Drugs targeting this process function by either mimicking endogenous prostaglandins at the EP receptor (e.g., Misoprostol) or by directly stimulating the expression of mucin genes (MUC5AC, MUC6) and the enzymatic pathways involved in glycoprotein assembly. These actions increase the volume and viscosity of the gastric mucus layer and promote the secretion of bicarbonate to maintain a neutral pH at the epithelial surface.
