Overview

The gastric mucosal barrier is a multifaceted defense system in the stomach that protects against self-digestion by acid and pepsin. It consists of a mucus layer rich in mucin glycoproteins and bicarbonate ions, secreted by specialized epithelial cells. The mucus traps water, excludes large molecules, and resists proteolysis, while bicarbonate neutralizes acid near the epithelial surface, maintaining a near-neutral pH despite an acidic lumen. Damage or dysfunction of this barrier can lead to gastritis or ulceration.

Mechanism of action

NSAIDs inhibit prostaglandin synthesis, reducing bicarbonate and mucin secretion. Cimetidine reduces acid secretion which indirectly can affect bicarbonate secretion. Misoprostol is a synthetic prostaglandin E1 analog that can increase mucin and bicarbonate secretion.
