Overview

Gastric refluxate is a complex mixture of hydrochloric acid, pepsin, bile acids, and pancreatic enzymes that retrogradely enters the esophagus from the stomach (StatPearls, 2023). It serves as the primary pathological agent in Gastroesophageal Reflux Disease (GERD), where its acidic and proteolytic nature causes mucosal damage and inflammation (NIH, 2022). Although not a single molecular target, the refluxate is the focus of various therapeutic interventions aimed at mitigating its harmful effects. Antacids work by directly neutralizing the hydrochloric acid within the refluxate, while alginates form a physical raft to prevent the mixture from ascending into the esophagus (PubMed, 2021). Additionally, drugs like Proton Pump Inhibitors (PPIs) indirectly target the refluxate potency by inhibiting the H+/K+ ATPase enzyme, thereby reducing its acid concentration (StatPearls, 2023). Chronic exposure to gastric refluxate is a significant risk factor for the development of Barrett esophagus and esophageal adenocarcinoma (NIH, 2022).
