Overview

Gastrointestinal lumen cations refer to the positively charged ions, primarily potassium (K+), sodium (Na+), and ammonium (NH4+), located within the fluid of the digestive tract [StatPearls, 2023]. While not a traditional protein-based receptor or enzyme, these ions serve as the direct therapeutic target for a class of medications known as cation exchange resins or sequestering agents [DrugBank, 2024]. These agents are designed to remain non-absorbed as they pass through the gut, where they bind specific cations in exchange for other ions like calcium, sodium, or hydrogen [FDA Label, 2015]. By sequestering these ions, the drugs prevent their systemic absorption or facilitate their removal from the body via fecal excretion [NEJM, 2015]. This therapeutic approach is primarily used to manage electrolyte disorders, most notably hyperkalemia in patients with chronic kidney disease or heart failure [JAMA, 2014]. The interaction between the drug and the luminal cations helps maintain systemic electrolyte homeostasis when renal excretion is impaired [PubMed, 2021].
