Gastrointestinal (GI) motility targets encompass a broad array of molecular entities, primarily G protein-coupled receptors (GPCRs), enzymes, and ion channels, that coordinate the mechanical activity of the gut [1, 11]. Key targets include the 5-Hydroxytryptamine receptor 4 (5-HT4), motilin receptor, ghrelin receptor, and dopamine D2 receptor, which modulate the release of excitatory neurotransmitters like acetylcholine or inhibitory ones like nitric oxide [2, 8]. These targets are essential for maintaining normal peristalsis, gastric emptying, and intestinal transit, and their dysregulation is central to the pathogenesis of various GI motility disorders, including gastroparesis, chronic idiopathic constipation, and irritable bowel syndrome (IBS) [5, 10]. Pharmacological modulation of these targets—using prokinetic agents to stimulate movement or antidiarrheal agents to slow it—is a cornerstone of clinical management [4, 6]. However, many drugs hitting these targets are associated with significant safety concerns, such as the risk of cardiac arrhythmias (QT prolongation) or neurological side effects (extrapyramidal symptoms), necessitating careful patient selection and monitoring [6, 10].
