Gastrointestinal mucosal inflammation is a pathological state characterized by the infiltration of inflammatory cells into the mucosal layer of the digestive tract, leading to tissue damage and impaired barrier function (StatPearls, 2023). This condition is a hallmark of Inflammatory Bowel Diseases (IBD), such as Crohn's disease and ulcerative colitis, where an aberrant immune response to gut microbiota occurs in genetically susceptible individuals (NIH, 2022). The inflammatory process involves the overproduction of pro-inflammatory cytokines like TNF-alpha, IL-12, and IL-23, which drive chronic tissue destruction (PubMed, 2021). Therapeutic intervention focuses on dampening this immune response through various classes of drugs, including biologics that neutralize specific cytokines or small molecules that inhibit signaling pathways like Janus kinases (Nature Reviews Gastroenterology & Hepatology, 2020). Achieving mucosal healing, defined as the resolution of visible and microscopic inflammation, is the primary goal of modern treatment to prevent long-term complications such as fibrosis and malignancy (Journal of Crohn's and Colitis, 2021). Monitoring the severity of inflammation is typically performed using endoscopic scores and non-invasive biomarkers like fecal calprotectin (Clinical Gastroenterology and Hepatology, 2022).
Therapeutic agents work by inhibiting pro-inflammatory cytokines (e.g., TNF-alpha, IL-12/23), blocking leukocyte trafficking via integrin inhibition, or modulating intracellular signaling through Janus kinase (JAK) inhibition.
