The gastrointestinal mucosal membrane is the innermost lining of the GI tract, composed primarily of epithelial cells, an underlying lamina propria, and a thin layer of smooth muscle called the muscularis mucosae[2]. It is covered by a mucus layer mainly secreted by goblet cells, consisting of mucins such as MUC2, MUC5AC, MUC5B, and MUC6[3][4]. This structure serves as a crucial physical and biochemical barrier, facilitating lubricated passage of food, protection from pathogens and acid, nutrient absorption, and immune surveillance[2][3][4]. Dysfunction or injury to the mucosal membrane underlies many GI diseases, including inflammatory, neoplastic, and infectious processes[3][4].
Drugs affecting the gastrointestinal mucosal membrane can act through various mechanisms including enhancement of mucus secretion or barrier function (e.g., sucralfate, misoprostol), inhibition of acid secretion (e.g., proton pump inhibitors), immunosuppression or anti-inflammatory effects (e.g., corticosteroids, mesalamine), or antibiotic/antimicrobial action on microbiota.
