Overview

The “gastrointestinal osmotic effect” describes the process by which non-absorbable or poorly absorbed solutes in the gut lumen create an osmotic gradient that draws water into the intestinal lumen. This effect increases bowel water content, decreases absorption of solutes and water, and can result in diarrhea if excessive. Common causes include malabsorption of carbohydrates (such as lactose or sorbitol) and the use of osmotic laxatives, which are drugs designed to deliberately trigger this effect to relieve constipation. The process is not mediated by a specific protein or receptor, but can be modulated by various physiological factors, such as intestinal motility, microbiota, aquaporin water channels, and the integrity of the mucosa. It plays a role in gastrointestinal disorders such as osmotic diarrhea, and it is leveraged therapeutically in the management of constipation
