Overview

Gastrointestinal smooth muscle calcium handling refers to the complex physiological process governing the elevation and reduction of cytosolic calcium ions (Ca2+) to regulate contraction and relaxation in the gut. This system integrates calcium influx through L-type voltage-gated calcium channels (e.g., CaV1.2) and T-type channels with internal release from the sarcoplasmic reticulum via inositol trisphosphate receptors (IP3R) and ryanodine receptors (RyR). Calcium removal is managed by the sarco/endoplasmic reticulum Ca2+-ATPase (SERCA) and the plasma membrane Ca2+-ATPase (PMCA), as well as the sodium-calcium exchanger (NCX). Dysregulation of these pathways is a hallmark of various motility disorders, such as irritable bowel syndrome (IBS) and gastroparesis, where abnormal calcium transients lead to uncoordinated peristalsis. Pharmacological intervention often targets specific components of this system, most notably L-type calcium channels, to alleviate spasms or enhance transit in the digestive tract.
