The gastrointestinal smooth muscle cell mitochondrial quality control (MQC) system is a complex regulatory network dedicated to maintaining the integrity and functionality of the mitochondrial network within the gut's contractile tissues (11). This system integrates three primary processes: mitochondrial biogenesis, which involves the creation of new mitochondria; mitochondrial dynamics, which balances organelle fusion and fission; and mitophagy, the selective degradation of damaged mitochondria (1, 4). In gastrointestinal smooth muscle cells, MQC is vital for providing the high energy levels required for rhythmic muscle contractions and peristalsis (6, 8). Dysregulation of this system, often driven by oxidative stress or metabolic imbalances, is a significant factor in the pathogenesis of motility disorders such as diabetic gastroparesis and chronic intestinal pseudo-obstruction (9, 10). While not a single molecular target, various components of the MQC system, such as PGC-1α and Drp1, are being explored as therapeutic focal points to restore gut motility and cellular health (4, 13). Pharmacological interventions typically aim to enhance biogenesis or inhibit excessive mitochondrial fragmentation to preserve smooth muscle function (1, 15).
