Overview

Gastrointestinal tract lumen osmolality is the measure of solute concentration (osmoles per kilogram of solvent) in the liquid contents of the GI tract. It plays a critical role in the movement of water and solutes across the gut wall, influencing both digestion and fluid homeostasis. Changes in GI lumen osmolality can be sensed and signaled neuro-hormonally to the brain, particularly influencing thirst and satiety mechanisms. Clinically, manipulation of GI osmolality underlies the therapeutic action of osmotic laxatives and can affect the absorption of orally administered drugs. Pathologically, abnormal GI osmolality contributes to conditions such as osmotic diarrhea and electrolyte disturbances.

Mechanism of action

Induction of osmosis: Compounds increase GI lumen osmolality, pulling water into the lumen and promoting laxation (osmotic laxatives). Modulation of GI fluid volume may alter drug absorption kinetics (certain drugs, e.g., atenolol, may have decreased absorption in hyperosmotic environments).
