Overview

General protein substrates including fibrin-like deposits refer to a broad class of pathological proteinaceous materials, primarily consisting of fibrin and its derivatives, that accumulate in tissues and the vasculature during chronic inflammation and oxidative stress (DrugBank DB14751). These deposits, often termed parafibrin or dense matted deposits (DMDs), are characterized by their resistance to endogenous fibrinolysis and their role in obstructing microcirculation and promoting tissue edema (Pretorius et al., 2013). They serve as the primary therapeutic target for exogenous proteolytic enzymes such as serratiopeptidase. These enzymes catalyze the hydrolysis of peptide bonds within these substrates, facilitating the dissolution of abnormal protein aggregates and fibrin clots. This enzymatic action helps to reduce inflammation, alleviate swelling, and improve the penetration of other therapeutic agents, such as antibiotics, into the affected tissues. The degradation of these substrates is particularly beneficial in treating conditions like post-operative edema, chronic venous insufficiency, and respiratory diseases with viscous secretions. By clearing these deposits, the drugs restore normal tissue permeability and facilitate the body's natural healing processes. While effective, targeting these substrates requires careful management to avoid excessive fibrinolysis and potential bleeding complications.
