Overview

Geranylgeranyl pyrophosphate (GGPP) is a vital 20-carbon isoprenoid intermediate produced within the mevalonate pathway, serving as a critical branch point for the synthesis of various terpenes and the post-translational modification of proteins (PubChem, CID 5280458). Its primary biological function is to act as a substrate for geranylgeranyltransferases, which covalently attach the geranylgeranyl moiety to the C-terminus of small GTPases such as Rho, Rac, and Cdc42 (PubMed: 11162660). This process, known as geranylgeranylation, is essential for the membrane localization and functional activation of these signaling proteins, which regulate the actin cytoskeleton, cell polarity, and vesicular trafficking (PubMed: 16182142). In clinical practice, GGPP levels are indirectly modulated by several major drug classes, including statins and nitrogen-containing bisphosphonates, which deplete the GGPP pool to exert anti-inflammatory, anti-resorptive, or anti-tumor effects (PubMed: 22003301). Furthermore, the development of direct geranylgeranyltransferase inhibitors (GGTIs) represents a more targeted approach to disrupting Rho-dependent oncogenic signaling in various cancers (PubMed: 25611105). Consequently, GGPP acts as a central metabolic effector whose availability determines the activation state of numerous essential cellular pathways involved in bone health, cardiovascular function, and oncology.

Mechanism of action

Depletion of the intracellular GGPP pool through inhibition of upstream mevalonate pathway enzymes (HMG-CoA reductase or Farnesyl pyrophosphate synthase) or competitive inhibition of the enzymes (Geranylgeranyltransferases) that utilize GGPP as a substrate.
