Glioma cancer stem cell (GCSC) antigens presented in peptide-MHC complexes are specialized therapeutic targets designed to eliminate the self-renewing, therapy-resistant subpopulation of cells within glioblastoma. These targets consist of short peptide fragments derived from proteins highly or uniquely expressed by GCSCs—such as MAGE-1, HER2, AIM-2, and Survivin—which are displayed on the cell surface by Major Histocompatibility Complex (MHC) molecules (Phuphanich et al., 2013, Cancer Immunology, Immunotherapy). By targeting these specific pMHC complexes, immunotherapies like dendritic cell vaccines (e.g., ICT-107) and peptide vaccines (e.g., SurVaxM) prime the patient's immune system to recognize and destroy the cells responsible for tumor initiation, progression, and post-treatment recurrence (Wen et al., 2019, Neuro-Oncology). This approach is critical because GCSCs are often resistant to conventional chemotherapy and radiation, serving as a reservoir for tumor regrowth. However, the effectiveness of targeting these complexes can be limited by the high degree of intratumoral heterogeneity and the immunosuppressive microenvironment of the central nervous system (Ahluwalia et al., 2023, Journal of Clinical Oncology). Successful therapeutic intervention requires the identification of immunogenic peptides that are both highly specific to the stem cell population and compatible with the patient's specific Human Leukocyte Antigen (HLA) profile.
