Globo H hexasaccharide antigen is a globo-series glycosphingolipid found predominantly on the outer membrane of various human cancer cells—including those from breast, prostate, ovarian, and pancreatic cancers—while being largely absent from normal adult tissues. Structurally, it is a complex hexasaccharide [Fucα(1–2)Galβ(1–3)GalNAcβ(1–3)Galα(1–4)Galβ(1–4)Glc] linked to ceramide at its terminal glucose. Its restricted expression pattern makes it an attractive target for anticancer immunotherapy, especially as a component in synthetic vaccines designed to elicit an immune response specifically against tumor cells expressing this epitope. Several clinical and preclinical studies have demonstrated that vaccination with synthetic forms of the Globo H hexasaccharide can induce specific IgM/IgG antibodies capable of recognizing and killing tumor cells through complement-dependent cytotoxicity. However, like many carbohydrate-based antigens, its low inherent immunogenicity poses challenges for therapeutic development unless addressed through chemical modifications or potent adjuvants. There are no approved drugs directly targeting this molecule yet; ongoing research focuses on improving vaccine efficacy and understanding its precise biological roles in tumor progression and immune evasion.
