Globotriaosylsphingosine, commonly known as Lyso-Gb3, is a deacylated derivative of globotriaosylceramide (Gb3) and a critical biomarker for Fabry disease, an X-linked lysosomal storage disorder. It accumulates significantly in the plasma and tissues of patients with deficient alpha-galactosidase A activity, often serving as a more sensitive indicator of disease severity and treatment response than Gb3 itself. Beyond its role as a biomarker, Lyso-Gb3 is a potent pathogenic mediator that promotes smooth muscle cell proliferation, podocyte injury, and chronic inflammation by activating pathways such as Notch1 and Toll-like receptor 4 (TLR4). It also acts as a competitive inhibitor of the residual alpha-galactosidase A enzyme, further exacerbating the metabolic defect. Therapeutic strategies for Fabry disease, including enzyme replacement therapy (ERT), chaperone therapy, and substrate reduction therapy (SRT), aim to reduce Lyso-Gb3 levels to mitigate multi-organ damage. Monitoring Lyso-Gb3 is essential for evaluating the efficacy of these treatments and identifying the impact of neutralizing anti-drug antibodies.
