Overview

The renal glomerulus is a microscopic cluster of capillaries in the kidney that performs the first step of filtering blood to form urine [1]. It acts as a highly selective semipermeable membrane, allowing the passage of water and small solutes while preventing the loss of essential plasma proteins and blood cells [2]. This filtration barrier is composed of the fenestrated capillary endothelium, the glomerular basement membrane, and the epithelial podocytes [3]. Damage to these components results in various glomerular diseases, such as glomerulonephritis or diabetic nephropathy, often manifesting as proteinuria or hematuria [4]. Although the glomerulus is an anatomical structure rather than a single molecular target, it houses numerous specific targets for pharmacological intervention, including the renin-angiotensin system and various transport proteins, which are modulated to manage blood pressure and slow the progression of kidney disease [5].
