Glutamate decarboxylase 65 (GAD65) peptide-MHC complexes are critical molecular targets in the pathogenesis of Type 1 Diabetes (T1D) and other autoimmune disorders. These complexes form when GAD65, a key enzyme in GABA synthesis, is processed by antigen-presenting cells and its derived peptides are loaded onto Major Histocompatibility Complex (MHC) molecules, particularly HLA-DR4 and HLA-DQ8. Autoreactive CD4+ T cells recognize these specific peptide-MHC (pMHC) combinations, leading to the targeted destruction of insulin-producing pancreatic beta cells. Therapeutic strategies, such as antigen-specific immunotherapy (ASI), aim to utilize these pMHC complexes or their constituent peptides to reprogram the immune system. By administering GAD65 or its immunodominant peptides in a tolerogenic context, researchers seek to induce GAD65-specific regulatory T cells (Tregs) that can suppress the destructive autoimmune attack and preserve residual beta-cell function.
