Glycoprotein A repetitions predominant (GARP), encoded by LRRC32, is a type I transmembrane protein highly expressed on regulatory T cells (Tregs) and platelets[5]. GARP acts as a docking receptor for latent TGF-β1 complexes, facilitating their cell-surface presentation and subsequent activation—primarily via integrin αVβ8 or αVβ6 interactions[2][3][6]. Through this function, GARP enables Tregs to activate and release TGF-β1, a potent immunosuppressive cytokine, which in turn downregulates immune responses and supports tumor immune evasion, fibrosis, and tissue homeostasis[2][4][5][6]. GARP is considered a novel therapeutic target, especially in cancer and autoimmunity, with biologics under investigation that block the GARP–TGF-β1 axis[2][5][6].
Antibodies or inhibitors can prevent GARP-mediated activation of TGF-β1, thereby enhancing anti-tumor immunity or modulating immunosuppression
