Glycosaminaminoglycans (GAGs) are large, negatively charged linear polysaccharides that are major components of the extracellular matrix in the nucleus pulposus, the central region of the intervertebral disc[1][2][4]. The main GAGs present include chondroitin sulfate, keratan sulfate, heparan sulfate, and hyaluronic acid, with aggrecan proteoglycan carrying many GAG chains. These molecules attract and retain water, conferring the high osmotic pressure necessary for the disc to resist compressive mechanical loads and maintain flexibility[1][4][5]. With degeneration, GAG synthesis decreases, resulting in loss of disc hydration, biomechanical dysfunction, and increased risk of lower back pain and intervertebral disc disease[2][5]. GAG content of nucleus pulposus is a clinical biomarker for disc degeneration, and MRI-based techniques are used for noninvasive assessment[5].
