Overview

The glycosaminoglycan (GAG) layer of the urothelium is a protective, mucinous coating on the inner surface of the bladder and urinary tract epithelium. It is composed of proteoglycans expressing heparan sulfate, chondroitin sulfate, and dermatan sulfate. This layer acts as a barrier against urine components, toxins, and pathogens, preventing their penetration into deeper bladder layers. Damage or deficiency of the GAG layer is implicated in conditions like interstitial cystitis/bladder pain syndrome, leading to increased permeability, inflammation, and pain. Therapies targeting the GAG layer aim to restore its integrity and function, often using exogenous GAGs like hyaluronic acid or chondroitin sulfate.
