Overview

Granulin (GRN) mRNA is the messenger RNA transcript that encodes progranulin (PGRN), a multifaceted secreted glycoprotein vital for maintaining lysosomal health, modulating neuroinflammation, and promoting neuronal survival [1, 2]. Mutations in the GRN gene, most commonly leading to haploinsufficiency, result in a critical deficiency of progranulin protein, which is a major genetic driver of frontotemporal dementia (FTD-GRN) [3, 4]. Therapeutic interventions targeting GRN mRNA focus on restoring progranulin levels through gene replacement therapies, such as PR006 and PBFT02, which introduce functional GRN DNA to generate mRNA, or through antisense oligonucleotides (ASOs) designed to increase the stability or translation of the endogenous transcript [5, 6]. Because progranulin also acts as a growth factor, therapeutic strategies must carefully calibrate mRNA expression to avoid potential oncogenic risks associated with protein overexpression [7]. Maintaining the precise balance of this transcript is essential for preventing neurodegeneration while avoiding pathological cell proliferation [2, 7].
