Overview

Group A Streptococcus (GAS) M protein and streptococcal protective antigen (Spa) are critical surface-exposed virulence factors of Streptococcus pyogenes. M protein is a dimeric coiled-coil protein that serves as the primary antiphagocytic molecule by binding host complement regulators, such as Factor H and C4b-binding protein, to inhibit opsonization (Fischetti, 1989, Clin Microbiol Rev). Streptococcal protective antigen is an additional surface protein identified as a potent immunogen capable of eliciting protective immune responses, particularly in specific virulent strains like M18 (Dale et al., 2011, Vaccine). Together, these proteins are the focus of vaccine strategies designed to generate high titers of opsonic antibodies to prevent GAS infections ranging from simple pharyngitis to life-threatening necrotizing fasciitis. However, the extreme sequence variability of the M protein N-terminus necessitates the development of multivalent or conserved-region vaccines to ensure broad coverage (Walker et al., 2014, Clin Infect Dis). A critical safety consideration in targeting these antigens is the avoidance of epitopes that share homology with human proteins, which could potentially induce autoimmune sequelae like acute rheumatic fever (Cunningham, 2000, Clin Microbiol Rev).
