Group A Streptococcus pyogenes antigens are a diverse array of molecules expressed by the Gram-positive bacterium Streptococcus pyogenes, which is responsible for a wide spectrum of human diseases ranging from mild pharyngitis to life-threatening necrotizing fasciitis (StatPearls: Streptococcus Pyogenes). The most prominent antigen is the M protein, a surface-anchored coiled-coil protein that inhibits opsonization by binding to host complement regulators and fibrinogen (UniProt: M protein). Other critical antigens include the Group A carbohydrate (GAC), which is a cell wall polysaccharide used for clinical classification, and secreted toxins such as streptolysin O (SLO) and streptococcal pyrogenic exotoxins (SpeA/B/C) that induce host cell lysis and hyper-inflammatory responses (PubMed: Virulence factors of Group A Streptococcus). These antigens serve as the primary targets for diagnostic assays, such as rapid antigen detection tests (RADT), and are the focus of global vaccine development efforts (WHO: Group A Streptococcus vaccine development Technology Roadmap). Therapeutic intervention often involves the use of intravenous immunoglobulins (IVIG) to neutralize circulating toxins or the development of subunit vaccines designed to elicit protective antibodies without inducing cross-reactive autoimmune responses against host tissues (Nature Reviews Microbiology: Pathogenesis and vaccine strategies for GAS).
