Overview

Gut mucosal integrity refers to the complex physical and functional barrier of the gastrointestinal tract that separates the internal environment from the luminal contents, including pathogens, toxins, and dietary antigens (PubMed, PMID: 33535250). This barrier is maintained by a multi-layered system consisting of a mucus layer, a single layer of epithelial cells joined by tight junction proteins (such as claudins and occludins), and the underlying gut-associated lymphoid tissue (GALT) (NIH, 2022). Disruption of this integrity, often referred to as increased intestinal permeability or "leaky gut," is a hallmark of several inflammatory and autoimmune conditions, including Crohn's disease and ulcerative colitis (StatPearls, 2023). While gut mucosal integrity is a physiological state rather than a single molecular target, it is a critical therapeutic endpoint. Drugs like Teduglutide, a glucagon-like peptide-2 (GLP-2) analog, are used to enhance this barrier by promoting mucosal growth and increasing absorptive capacity in conditions like short bowel syndrome (FDA, 2012). Other experimental approaches target specific components of the barrier, such as Larazotide acetate, which acts as a tight junction regulator to reduce permeability in celiac disease (ClinicalTrials.gov, 2021).

Therapeutic interventions aim to restore gut mucosal integrity by stimulating epithelial cell proliferation and repair (e.g., GLP-2 analogs), inhibiting tight junction disassembly (e.g., zonulin antagonists), or neutralizing pro-inflammatory cytokines like TNF-alpha that increase permeability (NIH, 2023).
