Overview

The Gut-Immune-Microbiome Axis is a complex, multi-component system involving the bidirectional communication between the host's intestinal microbiota, the epithelial lining, and the mucosal immune system (Belkaid & Hand, Science, 2014). Drugs targeting this system typically focus on specific molecular mediators, such as cytokines or integrins, to dampen aberrant immune responses or prevent the infiltration of inflammatory cells into the gut tissue. Additionally, emerging therapeutic strategies like fecal microbiota transplantation (FMT) and precision probiotics aim to directly reshape the microbial community to restore a healthy gut environment (Nielsen et al., Nature Microbiology, 2022). Because this is a biological system rather than a single molecular target, it represents a broad therapeutic landscape for treating autoimmune and inflammatory diseases.

Therapeutic modulation involves neutralizing pro-inflammatory cytokines (e.g., TNF-alpha, IL-23), inhibiting leukocyte trafficking to the gut mucosa (e.g., alpha-4-beta-7 integrin antagonism), or altering the microbial composition to restore metabolic and immune balance.
