Gastrointestinal motility refers to the coordinated contractions and relaxations of smooth muscle throughout the digestive tract that move food, liquids, and waste from the mouth to the anus. This process is essential for proper digestion and absorption. It is regulated by complex interactions between smooth muscle cells, enteric neurons within the enteric nervous system (ENS), hormones such as ghrelin and motilin acting via their respective G protein-coupled receptors on various cell types in the gut wall, autonomic input from both parasympathetic and sympathetic nerves, local reflexes within myenteric plexus circuits, chemical composition/volume/osmolarity of luminal contents, and other factors. Abnormalities in gastrointestinal motility can result from nerve damage or dysfunctions in muscular coordination leading to clinical disorders such as gastroparesis or irritable bowel syndrome. While many drugs can influence gastrointestinal transit time by acting on specific molecular targets like serotonin receptors or opioid receptors within this system—and thus are used therapeutically for conditions like constipation or diarrhea—“gastrointestinal motility” itself is not a discrete molecular entity but rather an emergent property arising from multiple interacting components at tissue/organ level.
