Overview

The gut mucosal innate immune system is a multi-layered defense network essential for maintaining intestinal homeostasis and protecting against enteric pathogens (Mowat & Agace, 2014). It consists of physical barriers like the intestinal epithelium and mucus layer, as well as biochemical defenses including antimicrobial peptides and secretory IgA (Okumura & Takeda, 2017). Specialized innate immune cells, such as dendritic cells, macrophages, and innate lymphoid cells (ILCs), utilize pattern recognition receptors (PRRs) to monitor the luminal environment and distinguish between commensal bacteria and harmful invaders (Neurath, 2014). When this system is dysregulated, it leads to chronic inflammation, a hallmark of diseases like Crohn's disease and ulcerative colitis (Ungaro et al., 2017). Therapeutic intervention typically involves targeting specific molecular pathways within this system, such as the TNF-alpha signaling pathway or the recruitment of lymphocytes via integrin receptors (Danese et al., 2015). By modulating these innate and early adaptive responses, drugs aim to restore the delicate balance between immune tolerance and active defense in the gut (Belkaid & Hand, 2014).

Mechanism of action

Drugs targeting the gut mucosal innate immune system function by neutralizing pro-inflammatory cytokines (e.g., TNF-alpha, IL-12, IL-23), inhibiting the migration of inflammatory cells into the gut tissue by blocking adhesion molecules (e.g., alpha-4 beta-7 integrin), or modulating intracellular signaling cascades like the Janus kinase (JAK) pathway to reduce the production of inflammatory mediators (Neurath, 2017; Sandborn et al., 2013).
