Overview

Hair follicle vasoconstriction is a physiological process characterized by the narrowing of blood vessels supplying the hair follicles, primarily within the dermal papilla. This process is not a single molecular target but a biological response typically mediated by the activation of alpha-1 adrenergic receptors on vascular smooth muscle cells (StatPearls, 2023). In a therapeutic context, inducing hair follicle vasoconstriction is utilized to prevent chemotherapy-induced alopecia (CIA). By reducing blood flow to the scalp during the peak plasma concentration of cytotoxic drugs, the delivery of these agents to the highly proliferative hair follicle cells is minimized, thereby reducing hair loss (Soref et al., 2015). This is achieved either through physical means, such as scalp cooling, or pharmacological means using topical alpha-adrenergic agonists like phenylephrine (Journal of the American Academy of Dermatology, 2019). The strategy aims to create a temporary vascular shield to protect the hair follicle from systemic toxicity. Effective induction of vasoconstriction requires precise timing relative to chemotherapy administration to ensure that blood flow is restricted when drug levels are highest. While primarily used for CIA, the study of follicular blood flow also has implications for other forms of alopecia where microcirculation is compromised. Monitoring of this process often involves measuring scalp temperature or using laser Doppler flowmetry to confirm reduced perfusion. Safety considerations include avoiding excessive ischemia that could lead to tissue damage or systemic effects from absorbed agonists.
