Overview

Heartland virus (HRTV) is an emerging tick-borne pathogen in the family Phenuiviridae, primarily transmitted by the Lone Star tick (Amblyomma americanum) in the United States. It possesses a tri-segmented, negative-sense RNA genome that encodes several proteins, including an RNA-dependent RNA polymerase, glycoproteins, and a non-structural protein (NSs). HRTV causes Heartland virus disease, a severe febrile illness characterized by fever, fatigue, thrombocytopenia, and leukopenia, often requiring hospitalization and potentially leading to multi-organ failure. The viral NSs protein acts as a critical virulence factor by antagonizing host innate immunity, specifically by blocking the TBK1-IRF3 interaction and inhibiting STAT-mediated interferon signaling. There are currently no FDA-approved vaccines or specific antiviral therapies, making management primarily supportive. Recent preclinical research has identified the viral RNA polymerase as a key therapeutic target, with nucleoside analogs like 4'-fluorouridine (EIDD-2749) showing significant efficacy in mouse models. Other experimental agents, such as favipiravir and the antifungal anidulafungin, have also demonstrated inhibitory activity against the virus in laboratory settings.
