Heat shock protein beta-6 (HSPB6), widely known as HSP20, is a small heat shock protein that functions as a key regulator of smooth muscle relaxation and cytoskeletal stability. It is predominantly expressed in cardiac, skeletal, and smooth muscle tissues, where it acts as a molecular chaperone and a signal transducer (UniProt: O14558). The downstream cytoskeletal pathway of HSP20 is primarily activated by its phosphorylation at the Serine 16 residue by cAMP-dependent protein kinase (PKA) or cGMP-dependent protein kinase (PKG). Once phosphorylated, HSP20 interacts with actin-binding proteins such as cofilin and the Arp2/3 complex, effectively modulating actin filament dynamics and preventing the formation of actin-myosin cross-bridges (PubMed: 15618525). This mechanism is vital for inducing vasodilation and bronchodilation, positioning HSP20 as a therapeutic target for cardiovascular diseases like hypertension and vasospasm, as well as respiratory conditions like asthma (PubMed: 22547407). Additionally, HSP20 provides significant cardioprotection by inhibiting apoptosis and reducing ischemia-reperfusion injury in the heart (PubMed: 25650345). Experimental therapies include the use of cell-permeant phosphopeptide mimetics, such as AZX100, that bypass upstream signaling to directly activate the downstream cytoskeletal effects of HSP20.
Phosphorylation at Ser16 by PKA or PKG leads to the dissociation of HSP20 from large aggregates, allowing it to interact with cytoskeletal proteins like cofilin and actin, which inhibits actin-myosin cross-bridge formation and promotes smooth muscle relaxation.
