Helicobacter pylori cell surface and enzyme systems represent a complex network of molecular targets essential for the survival, colonization, and pathogenicity of the bacterium within the human gastric mucosa. A central component is the enzyme urease, which catalyzes the hydrolysis of urea into ammonia and carbon dioxide, effectively neutralizing local gastric acid to create a habitable microenvironment (StatPearls, 2023). The cell surface features various adhesins and flagella that enable the bacterium to penetrate the mucus layer and adhere to epithelial cells, while specialized secretion systems deliver virulence factors like CagA and VacA into host cells to manipulate signaling and induce inflammation (Nature Reviews Microbiology, 2006). Therapeutic intervention typically involves a combination of antibiotics that target these systems, such as beta-lactams inhibiting cell wall synthesis and macrolides blocking protein translation (Journal of Clinical Medicine, 2020). The primary clinical objective of targeting these systems is the eradication of the infection to mitigate the risk of chronic gastritis, peptic ulcers, and gastric adenocarcinoma. However, the increasing prevalence of antibiotic-resistant H. pylori strains remains a major therapeutic challenge, necessitating the development of novel inhibitors targeting specific enzymes like urease or carbonic anhydrase (WHO, 2017).
