Overview

The Helicobacter pylori outer cell envelope is a complex, multi-layered structure essential for the survival and pathogenicity of the bacterium within the human gastric environment (Kusters et al., 2006, Clinical Microbiology Reviews). It comprises an inner cytoplasmic membrane, a periplasmic space containing a thin peptidoglycan layer, and a specialized outer membrane (Salama et al., 2013, Nature Reviews Microbiology). This envelope acts as a robust physical barrier against gastric acid and host immune responses while facilitating the transport of essential nutrients. Key components include lipopolysaccharides (LPS) and a variety of outer membrane proteins (OMPs), such as BabA and SabA, which mediate high-affinity binding to the gastric mucosa (Doig et al., 1999, Journal of Biological Chemistry). As a therapeutic target, the envelope is the site of action for several frontline antibiotics; for instance, amoxicillin targets the penicillin-binding proteins involved in peptidoglycan synthesis, leading to cell lysis (StatPearls, 2023). Bismuth-based compounds also interact with the envelope, causing structural degradation and inhibiting bacterial enzymes (Lambert et al., 1991, Reviews of Infectious Diseases). Furthermore, the envelope's low permeability and the emergence of mutations in its structural proteins pose significant challenges to effective treatment. These adaptations contribute to the rising global rates of antibiotic resistance, making the envelope a focus for novel drug and vaccine development (Mégraud et al., 2021, Gut). Understanding the molecular architecture of this envelope is critical for overcoming the protective mechanisms that allow H. pylori to persist for decades in the host.
