Hematopoietic reconstitution via allogeneic transplantation is the clinical process whereby donor hematopoietic stem cells (most commonly from bone marrow, peripheral blood, or umbilical cord) are infused into a recipient to regenerate the recipient's hematopoietic and immune systems. This process requires matching donor and recipient tissues (primarily HLA typing), robust immunosuppression to prevent rejection and GVHD, and is utilized for conditions such as hematologic malignancies, severe immune deficiencies, and certain genetic disorders. Unlike conventional drug targets, it is an orchestrated therapeutic procedure involving multiple cell types, molecular interactions, and immune mechanisms rather than a single target or pathway.
The mechanism involves significant immunosuppression to prevent rejection and graft-versus-host disease (GVHD), myeloablation to eliminate the recipient's bone marrow prior to transplant, and relies on the graft-versus-leukemia effect where donor immune cells target residual malignant cells.
