Hematopoietic stem and progenitor cell (HSPC) engraftment refers to the process by which transplanted hematopoietic stem cells (HSCs) and their progenitors migrate to the bone marrow, proliferate, and reconstitute the recipient’s blood-forming system. This process is fundamental for successful hematopoietic stem cell transplantation (HSCT), used for treating blood cancers, immune deficiencies, autoimmune diseases, and hereditary disorders. Successful engraftment restores normal hematopoiesis, while failure can result in life-threatening complications. Factors affecting engraftment include the quality/quantity of infused cells, recipient conditioning, donor-recipient compatibility, and microenvironmental signals. Major risks include graft failure, GVHD, and infection.
