Hemoglobin-based oxygen carriers are engineered molecules (often derived from bovine hemoglobin) designed to transport oxygen in the bloodstream when red blood cells are unavailable or contraindicated[5][6][1]. Used as blood substitutes or “oxygen therapeutics,” they have shown efficacy in acute anemia, trauma, and ischemic conditions by enhancing oxygen delivery to hypoxic tissues and supporting organ function until recovery or definitive therapy[3][5][2]. HBOCs present specific safety concerns not seen with natural hemoglobin inside red cells, notably vasoconstriction, increased blood pressure, and risk of methemoglobinemia and organ toxicity[1][3][4][5]. Innovations include chemical modifications (e.g., polymerization, cross-linking, surface modifications) to enhance stability, oxygen off-loading, and compatibility, as well as reduce toxicity[1][5][4]. HBOCs are compatible with all blood types, can be stored at room temperature for long periods, and do not carry infectious agents or cell antigens, making them valuable in emergencies, perioperative management, or for patients refusing blood transfusions[3][5][2].
Delivers oxygen directly to tissues by mimicking natural hemoglobin oxygen-binding and release. Facilitates oxygen transfer from red cells to hypoxic tissue in settings of acute anemia or impaired red cell circulation. Nitric oxide scavenging (can affect vascular tone).
