The Hemoglobin subunit gamma-1 (HBG1) promoter is a critical DNA regulatory region that controls the expression of the A-gamma globin chain, a key component of fetal hemoglobin (HbF) (UniProt P69891). During the transition from fetal to postnatal life, this promoter is transcriptionally silenced by the recruitment of repressors like BCL11A and ZBTB7A, leading to the hemoglobin switch from HbF to adult hemoglobin (PubMed: 31167780). In patients with sickle cell disease or beta-thalassemia, the HBG1 promoter serves as a high-value therapeutic target for gene-editing interventions (NIH, 2023). By utilizing CRISPR/Cas12a or base editing to introduce mutations or deletions in the repressor-binding motifs of the promoter, the silencing mechanism can be bypassed, leading to the re-induction of HbF (Editas Medicine, 2024). This therapeutic reactivation of HbF compensates for defective adult hemoglobin, preventing the polymerization of sickle hemoglobin and improving erythropoiesis in thalassemic patients (Beam Therapeutics, 2024). Consequently, the HBG1 promoter represents a pivotal site for curative genomic interventions in hematology.
