Hemostatic mechanical matrices are medical devices or biomaterials used to control bleeding by providing a physical scaffold that promotes blood clot formation. They work by absorbing blood and concentrating platelets and clotting factors at the site of bleeding, facilitating the body's natural hemostatic processes. Common types include gelatin sponges (e.g., Gelfoam, Surgifoam), microfibrillar collagen (Avitene), and microporous polysaccharide spheres (Arista). These agents are particularly useful when conventional methods are insufficient or when coagulopathies are present, and some may be used in combination with topical thrombin or fibrin sealants for enhanced clot formation. While generally safe and effective, complications such as foreign body reactions, infection, delayed resorption, or local tissue compression can occur if not used properly. These products do not occupy a molecular pathway targeted by drugs but rather provide a platform for physiologic hemostasis through mechanical means.
