Heparan sulfate and heparin are complex, linear glycosaminoglycans (GAGs) composed of repeating disaccharide units that are highly sulfated and negatively charged. Heparan sulfate is ubiquitously expressed on cell surfaces and within the extracellular matrix as part of proteoglycans, where it regulates various physiological processes including cell-cell signaling, tissue repair, and inflammation by interacting with growth factors and cytokines (NCBI Bookshelf, 2022). Heparin, a more highly sulfated variant primarily stored in mast cell granules, is widely utilized as a clinical anticoagulant due to its potent ability to catalyze the inhibition of coagulation proteases by antithrombin III (PubMed, 2023). In disease states, these molecules are implicated in cancer metastasis through the action of heparanase, and they serve as critical attachment factors for numerous viruses, including SARS-CoV-2 and herpes simplex virus (Nature Communications, 2020). Pharmacologically, heparin and its low-molecular-weight derivatives are mainstays in treating thromboembolic disorders, while their neutralization by agents like protamine sulfate is essential in surgical settings (StatPearls, 2023). Beyond anticoagulation, research into heparan sulfate mimetics and heparanase inhibitors continues to explore their potential in oncology and inflammatory diseases (Journal of Medicinal Chemistry, 2021).
Heparin and heparan sulfate interact with a wide array of proteins, most notably antithrombin III, which they activate to inhibit thrombin and Factor Xa, thereby preventing blood clot formation. They also function as co-receptors for growth factors and chemokines, modulating signal transduction pathways, and serve as attachment sites for pathogens during the initial stages of infection.
