Overview

Heparin cofactor II (HCII) is a plasma glycoprotein and a member of the serine protease inhibitor (serpin) family. It functions primarily as a specific inhibitor of thrombin (Factor IIa), with its inhibitory activity greatly accelerated in the presence of certain glycosaminoglycans, notably dermatan sulfate and heparin. HCII regulates extravascular thrombin activity and plays a role in inflammation and wound healing. Deficiency is rare and may increase thrombosis risk when combined with other risk factors.

Mechanism of action

Inhibits thrombin by forming a stable equimolar complex, blocking its active site and disrupting exosite I.
