Hepatic gluconeogenesis enzymes are a group of key metabolic enzymes in the liver responsible for synthesizing glucose from non-carbohydrate precursors such as lactate, glycerol, and glucogenic amino acids. This process is essential for maintaining blood glucose levels during fasting or starvation and is tightly regulated by hormonal signals including insulin, glucagon, cortisol (glucocorticoids), and thyroid hormones. Key enzymes include Pyruvate Carboxylase (PC), Phosphoenolpyruvate Carboxykinase (PEPCK), Fructose-1,6-bisphosphatase (FBPase), and Glucose-6-phosphatase (G6PC). Dysregulation leads to metabolic disorders: excessive activity contributes to hyperglycemia in diabetes mellitus; deficiencies can cause hypoglycemia. Targeting hepatic gluconeogenic enzymes—especially PEPCK or G‐6‐P—has been proposed for treating type 2 diabetes due to their role in inappropriate endogenous glucose production seen with insulin resistance.
