Hepatocyte nuclear factor 4 alpha (HNF4A) is a master regulatory transcription factor and a member of the nuclear receptor superfamily (NR2A1) that is essential for the development and functional maintenance of the liver, kidney, and intestines (UniProt P41235). It governs a complex gene network in hepatocytes, controlling the expression of genes involved in lipid transport, gluconeogenesis, and the synthesis of serum proteins like albumin and coagulation factors (PMID: 29127255). Mutations in the HNF4A gene are the primary cause of Maturity-Onset Diabetes of the Young type 1 (MODY1), a form of monogenic diabetes characterized by progressive beta-cell dysfunction and impaired insulin secretion (PMID: 11017077). In therapeutic contexts, HNF4A is considered a target for metabolic diseases and liver failure, as its downregulation is frequently associated with cirrhosis and hepatocellular carcinoma (PMID: 30639453). While it was long considered an orphan receptor, it is now known to constitutively bind fatty acids, and synthetic small molecules like BI6015 and Alverine have been identified as modulators to either inhibit or enhance its transcriptional activity for treating metabolic or malignant conditions.
HNF4A acts as a master transcriptional regulator by binding to specific DNA response elements as a homodimer. Its activity is modulated by the binding of fatty acid ligands (such as linoleic acid) to its C-terminal ligand-binding domain, which induces conformational changes that facilitate the recruitment of co-activators or co-repressors, thereby controlling the expression of a vast network of genes essential for hepatocyte function (UniProt P41235).
