Overview

Hepatocyte replacement refers to therapeutic strategies aimed at substituting diseased or dysfunctional liver cells (hepatocytes) with functional hepatocytes, typically by transplantation. This approach partially restores hepatic metabolic, synthetic, and detoxification capacity in patients with inherited metabolic liver diseases, acute liver failure, or select conditions where full organ transplantation is undesirable or infeasible. Unlike liver transplantation, hepatocyte transplantation is less invasive, may be repeated, and does not require removal of the native organ, but its clinical efficacy is often limited by immune rejection, poor cell engraftment, and limited cell supply.

Mechanism of action

Functional replacement of diseased/dysfunctional hepatocytes with healthy ones; Restoration/compensation of lost metabolic and synthetic functions of the liver

03

Biological functions

Restoration of hepatic functionMetabolic detoxificationCorrection of metabolic defectsTemporary hepatic support
04

Disease associations

Metabolic liver diseases (e.g., Crigler-Najjar syndrome, urea cycle disorders, phenylketonuria)Acute liver failurePotentially as adjunct in chronic liver failure, but with lower efficacy
05

Safety considerations

Immune rejection of transplanted cellsLimited long-term engraftment and functionShortage of donor cells/organsRisk of transmitting infections from donor cellsRisk of vascular occlusion or embolism from cell infusion
