Overview

The hepatoportal nerve reflex is a physiological mechanism that plays a critical role in whole-body glucose homeostasis by sensing nutrient concentrations in the portal vein (Burcelin et al., 2000). When glucose levels rise in the portal vein, specialized sensors—including glucose transporter 2 (GLUT2) and sodium-glucose co-transporter 3 (SGLT3)—trigger afferent neural signals, primarily via the vagus nerve, to the central nervous system (Mithieux, 2014). The brain then coordinates an efferent response that enhances peripheral glucose uptake in skeletal muscle and suppresses hepatic glucose production, effectively priming the body for glucose disposal (Burdakov et al., 2011). This reflex is often impaired in metabolic diseases like type 2 diabetes and obesity, contributing to insulin resistance and hyperglycemia (Vahl et al., 2007). While the reflex itself is a physiological pathway rather than a single molecule, pharmacological agents such as GLP-1 receptor agonists and metformin are believed to exert some of their therapeutic effects by modulating components of this hepatoportal sensing system (Knauf et al., 2008). Targeting the individual receptors and transporters within this reflex remains a significant area of interest for metabolic drug development (Delaere et al., 2013).
