Heroin metabolites, primarily 6-monoacetylmorphine (6-MAM) and morphine, are the pharmacologically active products of heroin (diacetylmorphine) metabolism that serve as the functional targets for sequestration-based immunotherapies (Bremer et al., 2017, J Am Chem Soc). Heroin itself is a prodrug that is rapidly deacetylated in the blood to 6-MAM, a highly lipophilic metabolite that readily crosses the blood-brain barrier to produce intense euphoric effects (Stowe et al., 2011, J Med Chem). In the development of vaccines and monoclonal antibodies, these metabolites are treated as haptens—small molecules that, when conjugated to carrier proteins, can elicit a specific immune response (Raleigh et al., 2017, J Pharmacol Exp Ther). The therapeutic strategy involves generating antibodies that recognize specific epitopes on these metabolites while they are accessible in the plasma. By binding these molecules in the systemic circulation, the antibodies prevent them from interacting with mu-opioid receptors in the brain, thereby blunting the reinforcing effects of the drug and reducing the risk of fatal respiratory depression (Pravetoni & Comer, 2019, Trends Pharmacol Sci; Janda & Treweek, 2012, Nat Rev Drug Discov).
Immunological sequestration: High-affinity antibodies bind to heroin metabolites in the plasma, creating a drug-antibody complex that is too large to cross the blood-brain barrier, thereby preventing the drug from reaching its receptors in the central nervous system (Bremer et al., 2017).
