Overview

T-cell receptors (TCRs) specific for HIV-1 Envelope (Env)-derived peptide–MHC complexes are specialized immune receptors that recognize viral antigens presented on the surface of infected cells. These TCRs bind to short peptide fragments derived from the HIV-1 envelope glycoproteins, gp120 and gp41, which are displayed by Major Histocompatibility Complex (MHC) molecules, typically HLA-A*02:01. In the context of HIV-1 infection, these receptors are central to the CD8+ T-cell-mediated destruction of infected cells, although natural responses often fail to eradicate the virus due to viral escape and T-cell exhaustion. Therapeutically, these TCRs are being utilized in adoptive cell therapies, such as TCR-engineered T-cell (TCR-T) therapy, and in the design of soluble TCR-bispecific molecules like ImmTAVs. These approaches aim to redirect the patient's immune system to target and eliminate the latent HIV reservoir, which remains the primary barrier to a functional cure. Unlike chimeric antigen receptors (CARs) that bind to surface-expressed proteins, Env-specific TCRs can recognize intracellularly processed viral proteins, providing a broader range of potential targets. However, the extreme sequence diversity of the HIV-1 envelope necessitates the identification of highly conserved epitopes to prevent viral resistance. Safety considerations include the risk of off-target effects if the TCR cross-reacts with human self-peptides and the potential for systemic inflammatory responses.
