HLA class II histocompatibility antigen DR beta 5 chain (HLA-DRB5) is a beta chain component of the MHC class II molecule, encoded by the HLA-DRB5 gene on chromosome 6. It forms a heterodimer with the alpha chain (HLA-DRA) and is expressed on antigen-presenting cells such as B lymphocytes, dendritic cells, and macrophages. HLA-DRB5 presents extracellular peptides to CD4+ T cells, thereby playing a pivotal role in adaptive immune responses. The region is highly polymorphic, affecting peptide binding and immune specificity; these polymorphisms are key in transplantation biology, autoimmune susceptibility (notably in multiple sclerosis), and adverse drug reactions. HLA-DRB5 is linked to the broader immunogenetic landscape influencing immune response variability, graft compatibility, and disease risk.
