Overview

Homeobox (HOX) transcription factors are a superfamily of master regulatory proteins containing a conserved 60-amino acid DNA-binding homeodomain. They play fundamental roles during embryonic development by orchestrating morphogenesis, cell differentiation, and the establishment of the body plan along the anterior-posterior axis. In adults, their expression is typically tissue-specific and restricted, but their dysregulation—particularly the overexpression of clusters like HOXA and HOXB—is a hallmark of various malignancies, including acute myeloid leukemia (AML) and solid tumors such as prostate, breast, and lung cancer. These proteins often function as oncogenes by promoting cell survival, proliferation, and angiogenesis, frequently through interactions with cofactors like PBX and MEIS. Although traditionally considered 'undruggable' due to their nuclear localization and lack of small-molecule binding pockets, therapeutic strategies now include peptide inhibitors like HTL001 that disrupt protein-protein interactions and Menin-MLL inhibitors that epigenetically suppress HOX gene expression. Additionally, specific homeobox members serve as critical biomarkers for patient stratification, such as the use of HOXC6 and DLX1 in prostate cancer diagnostic panels.
