Overview

The host gastrointestinal immune system is a vast and complex network of lymphoid tissues, immune cells, and signaling molecules that reside within the digestive tract. It represents the largest component of the human immune system, primarily organized into the gut-associated lymphoid tissue (GALT), which includes Peyer's patches, mesenteric lymph nodes, and the lamina propria (Mowat & Agace, 2014). Its primary biological function is to maintain homeostasis by distinguishing between harmless commensal microbiota and food antigens—a process known as oral tolerance—while mounting robust defenses against pathogenic microorganisms (NIH, 2023). Dysregulation of this system is central to the pathogenesis of chronic inflammatory conditions such as Crohn's disease, ulcerative colitis, and celiac disease (Neurath, 2014). Therapeutic intervention typically involves modulating specific molecular components of the system, such as inhibiting pro-inflammatory cytokines like TNF-alpha and IL-12/23 or blocking the trafficking of leukocytes to the gut mucosa using integrin antagonists (MDPI, 2023). Because of its critical role in both local and systemic immunity, the gastrointestinal immune system is a major focus for the development of immunomodulatory and microbiome-based therapies.
